This is the hit request form. You fill this form out and then send it to me. You, the sender, and I, the hitman, will be the only ones who knows who the target is, unless of course you tell someone.

Target name:
Reason for contract: If it's cuz someone pissed you off, sorry. Go somewhere else, or do it yourself. Guards prolly won't like it though.
Requested style: Ex. gangland, mafia, black ops, in your face, etc etc.
Special message: This is optional and can be anything such as a rose on the corpse, a saying to tell them before they die, or anything like that, but be careful, this could give away who ordered the hit.
Other info: Anything else you may want included. Don't go overboard however, as this limits my ability to be creative.
